In order to get a copy of your Medical Record, or to request that we send a copy to another party, please take the following steps.
- Complete and return the attached Tamber Authorization for Release of Medical Records
- Call us at (612) 814-0814 in order to pre-pay the copying fee.
The copying fee is based on our staff time, reproduction costs, handling costs, and mailing costs.
We will deliver the copy of the Medical Record 15 days after we receive the completed Release of Information and the copying fee.
